Schedule a consultation today to meet with Dr. Zager and we can explain step by step what our team can do to improve your oral health. We are proud to offer you flexible appointments; our offices are open Monday through Friday 8am -5pm and on Saturdays 8am – 2pm. We are committed to giving punctual service and seeing patients on a timely manner. Most patients arrive 10 minutes before the appointment to complete necessary forms and get answers to questions about payment and insurance. Anyone in pain or in and emergency situation will be seen as soon as possible.

Pease assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip from your dentist and any X-rays if applicable
  • A list of medications you are presently taking with your physician’s office number
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

Important

All patients under the age of 18 years of age must be accompanied by a parent or guardian to all visits. This person must be present in the office at the time of your surgery.

A pre-operative consultation and physical examination are mandatory for patients undergoing IV/General anesthesia for surgery. Please have nothing to eat or drink 6 hours prior to your surgery. You will also need an adult to drive or accompany you home.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, radiation, autoimmune therapy, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

X-Rays

If your dentist or physician has taken x-rays or special MRI/CT scans, you may request that they forward them or obtain copies for our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
